Construction Notice
As of August 13, 2024, construction of our new 10,000 square foot Outpatient Rehab will begin at Hôtel-Dieu Grace Healthcare. During this time individuals can expect closures of the main TNI entrance, as well as parking and drop off immediately outside of the TNI entrance. Patients, staff and visitors are encouraged to use Lot B for parking, including accessible parking spots and the Dr. Tayfour Rehabilitation entrance to access services in the TNI building, as well as CPH building. In addition to this, access to the first floor of the CPH Building will be restricted.

Hôtel-Dieu Grace Healthcare’s (HDGH) Concurrent Disorders Treatment Program, located on the CPH floors of the Toldo Neurobehavioural Institute, is a mental health and addictions outpatient program designed for any resident of Ontario, 16 years of age or older, whose health is affected by a diagnosed mental health concern and substance related misuse/dependency.
Integrated Treatment Approach
The Concurrent Disorders Treatment Program offers an evidence-based, DBT-informed, integrated approach, which means that mental health and addictions are treated simultaneously by trained clinical social workers. A psychiatry consult is available upon request.
Treatments are available and sensitive to culture, ethnicity, and gender.
Goals
The goal of the Concurrent Disorders Treatment Program is to work with you to design a personalized treatment plan that identifies your needs and the complex relationship between mental health and addiction.
The program is set up to allow for structure and maintain recovery while continuing to function in your natural environment.
Referrals
You may be referred to the Concurrent Disorder Treatment Program by another hospital, psychiatrist or physician, Windsor Addiction Assessment and Referral Services, or community mental health providers.

Cost
The Concurrent Disorders Treatment Program at HDGH is funded by the Ministry of Health and Long-Term Care. Services are provided at no cost to you upon presentation of a valid OHIP card.
Central Registration
Upon arrival to HDGH please present to the Central Registration Department, prior to proceeding to your appointment. Central Registration is located within the main entrance, across from the Courtyard Café.
Please arrive 15 min before your appointment in order to allow enough time to register.

Families’ and Friends’ Role in Promoting Safety
Family members and friends play an important role in patient safety. Encouraging
attendance of appointments, taking medications and asking to be involved in
understanding and facilitating treatment plans is recommended among family
members and friends. If family members and friends have serious concerns
about a loved one’s safety and well-being please utilize emergency (911) or
crisis hotline (519-973-4435) services or locations (nearest emergency room or
crisis centre) as needed. You are always allowed to provide us with
information however we may not be able to relay information/feedback to you
