Geriatric Assessment Program (GAP)
The Geriatric Assessment Program (GAP) at Hôtel-Dieu Grace Healthcare (HDGH) located in the Dr. Fouad Tayfour Regional Rehabilitation Centre (Tayfour Building) Casgrain, provides in-home comprehensive assessment, recommendations, consultation, caregiver education and support. This includes but is not limited to identifying, monitoring and managing dementia.
GAP services the healthcare needs of the senior population, typically 65 years or older, of Windsor and Essex County with complex health issues and/or a decline in their ability to function in their home.
Goals
The goals of GAP are to optimize the health, function, and independence of seniors with complex health concerns.
Clients typically have a number of chronic health conditions and present with an unexplained decline in health and/or functioning. Deterioration may either be due to changes in physical, cognitive health or psychosocial environment, or a combination of the three; threatening independence in daily living.
We aim to include the client’s support system throughout the assessment and consultation process.
Your inter-professional team will work together to address your health issues such as forgetfulness, confusion, medication concerns, weight loss/gain, walking difficulties/falls, bladder/bowel problems, loss of ability to care for oneself, caregiver stress, personality/behaviour changes, or driving concerns.
Referrals
Referrals to GAP must be made by a physician.
Cost
GAP services at HDGH are funded by the Ministry of Health and Long-Term Care. Services are provided without direct costs upon presentation of a valid OHIP card.
Central Registration
Upon arrival to HDGH please present to the Central Registration Department, prior to proceeding to your appointment. Central Registration is located within the main entrance, across from the Courtyard Café.
Please arrive 15 min before your appointment in order to allow enough time to register.
