Rehabilitation Outreach Team
The Rehabilitation Outreach Team at HDGH provides a personalized approach to support stroke survivors and their families in their community as they continue their rehabilitation journey. Rehabilitation Outreach services are offered based upon your individual needs and rehabilitation goals. Services are offered for a maximum of 12 weeks.
Goals
The Rehabilitation Outreach Team will utlize special assessment tools for those who are recovering from a stroke. Through information provided by yourself, your family, and other healthcare professionals, we create goals based on what is important to you. Together, we create a plan for you to be able to do the things or activities that you enjoy.
Referrals
Referrals to the Rehab Outreach Program must be made by a physician or nurse practitioner.
Cost
Rehabilitation Outreach Team services at HDGH are funded by the Ministry of Health. Services are provided without direct costs upon presentation of a valid OHIP card.
