Your Care Team
There are a number of individuals who may be included in your care while at Hôtel-Dieu Grace Healthcare (HDGH).
All staff, physicians and volunteers at HDGH wear a hospital identification badge, which includes their name, department and picture. If you are unsure about any person with whom you are interacting, the hospital encourages you to ask for identification, or if necessary, call a member of your care team for assistance.
Members of your care team may include:
Attending Physician or Most Responsible Physician (MRP)
Attending Physician or Most Responsible Physician is a doctor who is in charge of your care while you are in hospital. They also supervise the Fellow, Residents, or medical students who may also be involved in your care.
Bioethicist
HDGH provides Ethics Consultation Services to our staff, patients, and their families. Ethics Consultation Services assist patients, families, hospital staff, physicians, volunteers, and students on a wide range of ethical issues across the lifespan. Frequently encountered issues include value conflicts around decisions such as withholding or withdrawing treatment, consent and capacity, substitute decision-making, Do Not Resuscitate (DNR) orders, patient discharges, resource allocation, and organizational ethics. If you have questions or concerns that you feel would benefit from an ethics consult, please contact our Patient Advocate at extension 74404 or Patient.Advocate@hdgh.org to arrange services.
Certified Rehab Assistant
A Certified Rehab Assistant will provide therapy assistance to patients under the supervision of Physicial Therapists and Occupational Therapists.
Consulting Physician
A Consulting Physician is a specialist doctor who is asked by the Attending Physician to give recommendations about specific aspects of your care.
Dietitian
Dietitians help you choose the right foods for you meal planning that align with your care needs.
Environmental Services Worker
Environmental Services Workers ensure your room and the entire hospital remain clean, tidy, and disinfected to protect your health. Numerous items in your room will be cleaned daily.
Essential Care Partner (ECP)
An Essential Care Partner (ECP) is someone who has a close and trustworthy relationship with the patient and is actively involved in the patient’s day-to-day care, care planning and decision making. Patients at HDGH have the option of choosing an Essential Care Partner to assist them during their time at the hospital.
Geriatrician
Geriatricians are primary care doctors who have additional training in treating older adults, especially those 65 years and older.
Infection Control Practitioner
An infection control practitioner (ICP) helps to prevent and manage infections by ensuring infection prevention and control best practice guidelines and processes are followed. They work with the rest of the care team to reduce infection risks to protect patients, visitors and staff
Nurse Practitioner
A Nurse Practitioner has additional education and specialized training to assess and manages your medication needs and plan your care. They can make diagnoses, order tests, interpret results, and prescribe medications.
Occupational Therapist (OT)
Occupational Therapists help you plan how to safely do everyday activities such as eating, bathing, and getting dressed.
Patient Advocate
HDGH’s Patient Advocate welcomes feedback such as compliments, inquiries, suggestions, and complaints regarding the services we provide. The Patient Advocate facilitates resolutions of concerns, assists patients and families more through different services, supports a culture of quality and patient safety, and receives compliments that can be passed along to the care team. Personal Support Worker (PSW)
Personal Support Workers will assist you with basic care needs, as well as ensuring you are comfortable. The PSW also assists with other duties to ensure the nursing unit is well supported and running smoothly.
Pharmacist
Pharmacists will review and assure that all medications you are taking while at HDGH are appropriate for your care needs.
Physiatrist
A Physiatrist is a doctor who specializes in physical medicine and rehabilitation.
Physical Therapist (PT)
Physical Therapists help you plan how to be more independent by building your strength, balance and coordination.
Physician Assistant (PA)
A Physician Assistant is a medically educated clinician who works with the doctors. They help assess and manager your health and medical needs while you are in hospital.
Psychiatrist
A Psychiatrist is a medical doctor with additional training focused on the diagnosis, treatment, and prevention of mental, emotional, and behavioural disorders.
Registered Nurses (RN/RPN)
Registered Nurses or Registered Practical Nurses keep track of your health and well-being, and teach you about your illness or injury. Each day, you may have 2 to 3 difference nurses.
Respiratory Therapist
Respiratory Therapists assess, perform medical tests and provide a wide range of treatments for cardiorespiratory disease. They provide airway and breathing support and educate patients about lung health.
Security Guards
Security Guards are on site 24 hours a day, 7 days a week. Security performs continuous patrols of the entire property and are readily available to ensure all types of risks are mitigated and the personal safety of all remains paramount. All staff, physicians, students, and volunteers at HDGH must wear an ID badge to properly identify who they are; you are encouraged to ask to see identification for those people you are unsure about.
Spiritual Health Team
HDGH’s Spiritual Health Team are available as a compassionate resource for patients and families, offering a number of spiritual health services including religious celebrations through prayer and rituals appropriate to the patient’s faith, make connections between patients and Spiritual Leaders of various faiths, offering Holy Communion, weekly Roman Catholic Mass and weekly Interreligious Prayer Services.
Social Worker
Social Workers provide emotional support, advocacy, information and system navigation services to patients and families, as they cope with the challenges brought on by illness, hospitalization and treatment. Social Workers also assist in Advance Care Planning which allows patients to express personal choices about how they wish to be treated in different health situations.
