- Follow health and safety policies to help maintain a safe environment.
- Clarify your caregiver/support person and include them in care planning.
- Help us improve our services by sharing your experiences and suggestions.
- Understand any financial responsibilities related to your care, including co-payments, and talk to a staff member if you have questions or need support.

The Declaration is a vision that articulates a path toward patient partnership across the health care system in Ontario. It describes a set of foundational principles that are considered from the perspective of Ontario patients and serves as a guidance document for those involved in our health care system.
