Complex Medical Care
Hôtel-Dieu Grace Healthcare’s Complex Medical Care (CMC) Program in the Dr. Y. Emara Centre for Healthy Aging and Mobility (Emara Building) provides restorative care for those individuals who no longer require services in an acute care hospital, and because of their care needs, are temporarily unable to return to the community.
A Physician and/or Nurse Practitioner will be assigned to you on admission to monitor your progress and attend to any medical needs during your stay. Our services include Physiotherapy, Occupational Therapy, Speech and Language Pathology, Nursing, Social Work, Psychology, Spiritual Care and Dietary.
Location
The CMC program is located on the 2nd (South) and 3rd (North, South) floors of the Dr. Y. Emara Centre for Healthy Aging and Mobility (Emara Building).
Goals
You will be an active participant in your CMC journey. Within this time limited program, your inter-professional healthcare team will assist you with your medical needs and work with you to improve your independence by increasing your strength, mobility, and endurance to better manage your daily activities.
